Primary Function & Major Responsibilities Position Summary: Provides palliative care inpatient assessments and follow-ups within the scope of practice as defined by regulatory bodies, functions in the capacity of an Attending Provider in collaboration with Physicians and other Providers. Provides advanced practice clinical expertise to patients and families in need of complex symptom management. Assists in daily telephone consultation and triage as needed to provide timely response to providers, patients/families and other hospice and palliative care team members. Develops staff and physician relationships that facilitate ongoing referrals.Major responsibilities: a. Develops a working diagnosis for patients and translates clinical and diagnostic information into a care planb. Orders and interprets diagnostic tests and order necessary procedures for patient referred to th! e service;c. Writes Prescriptions consistent with training and within the scope of license, collaboration agreement and certifications;d. Plans for, participates in and/or conducts family care conferences as appropriate and in coordination with the hospice and palliative care team;e. Provides care and services that reflect cultural sensitivities and developmental abilities of the population served;f. Prepares and maintains accurate medical records, charts and documents to support the clinical care provided and reimbursement for the services provided;g. Submits coding and billing information and necessary clinical records in a timely manner to clinic coding and administrative staff. Meets periodically with coding and management staff to review individual profiles and identify opportunities for improvement in documentation and coding;h. May occasionally provide care in a Hospice and outpatient clinic settingi. Participates in the call schedule for the Hospice and Palliative C! are Team.
Qualifications
Education and Certification: - Graduation from an accredited Clinical Nurse Specialist graduate education program,- Master’s degree in nursing.- Certification as a Clinical Nurse Specialist (CNS) by American Nurses Credentialing Center(ANCC) or accredited Nursing Association- Active or eligible for Minnesota R.N. license and current DEA certification.
Experience:
Minimum of two years experience as an RN with adults in a hospital, clinic, long term care facility or home care setting preferred.Special Knowledge , skills, abilities: -Ability to perform comprehensive assessment including history and physical examinations, identification of complex symptoms, psychological and social needs and other multidimensional needs associated with serious illness and end of life care;-Ability to order and interpret diagnostic tests and order necessary procedures for patient referred to the service;-Ability to develop a working diagnosis for patients and to translate the clinical and diagnostic! information into a plan of care;-Excellent knowledge of pharmacological principles, especially for frail patients and those with advanced or terminal illness;- Working knowledge of the precepts of hospice and palliative medicine and the desire to continue to learn;- Functional and clinical skills to act independently and proactively, yet collaboratively with the hospice and palliative care teams;- Ability to set priorities while problem solving using critical thinking so that best possible outcomes for patients and families occur;- Excellent communication skills such that high level trust and confidence is developed with patients and their families;- Ability to work consultatively with Attending Physicians, NPs, CNSs, and PAs of other specialties. This may include coordinating the palliative care services to patients;- Ability to incorporate cultural sensitivity and principles of cross cultural care into relationships with patients and hospital and program staff;- Ability ! to stand, walk or sit for an extended period of time. Reaching by exten! ding hand(s) or arm(s) in any direction. Finger dexterity required to manipulate objects with fingers rather than whole hand(s) or arm(s). Communication skills using the spoken word. Ability to see within normal parameters. Ability to hear within normal range;- Ability to transport oneself from and to multiple work sites/facilities;-Effective time management and problem solving skills;- Ability to utilize the electronic health record system, computerized physician order entry and other components of the EHR system;- Knowledge of coding/reimbursement processes, requirements and related issues.Regular/Temporary
