The Shelter Case Manager will work closely with the Housing First Case Manager(s); the overnight Shelter Team and other members of The Gathering Place's Case Management team to ensure ongoing quality care for Shelter Guests. The position will provide rehousing services and individualized support to those experiencing homelessness. A significant part of this position would be helping individuals become “Paper Ready” for housing.
The position provides access to a range of community-wide team members who work collaboratively to assess, treat and support guests. Flexible work hours and availability are required. The position requires a dynamic individual with significant relationship-building/engagement skills that can seamlessly link guests to community resources, housing, and healthcare services through a strengths-based approach.
This is a permanent position where the hours worked are primarily evenings.
Duties/Responsibilities:
Reporting to the Manager of Clinical & Community Supports the Shelter Case Manager is responsible for:
- Engage with Guests staying in the Shelter to determine immediate needs and identify appropriate supports.
- Review housing history, and identify barriers to obtaining and retaining housing.
- Develop housing goals with guests, following through with support needed (landlord mediation, housing search, securing funding, etc.) on an individual basis
- Engage effectively with individuals experiencing serious mental health/addictions issues, cognitive difficulties, brain injuries and/or physical health issues.
- Link guests with relevant internal or community services as needed, with the ability to assess the appropriateness of referrals based on the guest’s goals.
- Advocate for guests from a social justice lens in the face of barriers to service
- Document all guest-related activities accurately and in a timely fashion through a secure electronic database
- Build and maintain relationships with Guests while maintaining clear and consistent boundaries
- Provide de-escalation skills and remain vigilant for behaviours or actions that could become a safety concern for Guests, Volunteers or staff
- Must be available for flexible hours including weekdays, evenings, weekends and holiday coverage when required.
- Responsible for the safe, effective and efficient delivery of services to TGP Guests. Application of organizational policies and processes pertaining to guest and workplace safety is expected.
Qualifications and Skills:
- Degree in Social work, Nursing or other relevant health fields
- Minimum of two years of relevant experience
- Knowledge of available and relevant community resources for marginalized persons
- Sound relationship-building abilities and skills in individual support, conflict resolution, and empathetic listening
- Ability to work closely with team members and medical professionals and participate effectively as part of a larger multi-disciplinary primary healthcare team
- Demonstrate understanding and sensitivity to links between diversity and marginalization
- Strong interpersonal skills, ability to creatively build relationships with those who may be socially isolated
- Proficient and professional documentation
- Demonstrate skills in systems navigation
- Creative, adaptable problem-solving skills
- Clear Record of Conduct and Vulnerable Sector Check
